*All bookings require a valid credit card on file. The $70 deposit will be refunded upon attendance at the initial consultation. Failure to attend a scheduled New Patient Appointment will forfeit the deposit paid. Appointments can be booked, cancelled, and modified 24/7 through the online system. Promotional Credit is not valid for payment of the booking deposit or accepted for payment for missed appointments. The clinic does not schedule appointments via phone, email, or text. 24-hour notice is required for cancellation or rebooking of a New patient appointment. **New Patient $200 Promotional Credit Conditions
The New Patient Promotional Credit is available to New Patients*** at the Radix Pain & Rehabilitation Centre only. Current and previous patients are not eligible for this promotion.
Promotional Credit is limited to chiropractic treatment services provided by Dr. Carter Campbell @ the Radix Rehabilitation Centre only and used in conjunction with personal health benefits to cover portions not covered. Promotional Credit can be used in conjunction with health benefit plans. Direct billing is available.
Promotional Credit is not valid for payment of the booking deposit or accepted for payment for missed appointments.
Promotional Credit can be used to a maximum of $100 per transaction. Unused balances will carry over to be used for any future appointments with Dr. Carter Campbell.
Promotional Credit has no cash value and expires 365 days after the date of the initial consultation. Promotional Credit cannot be redeemed for cash. Unused balances may be transferred to a friend or family member with the same limitations applied to the credit balance being transferred.
***New patients are considered patients who have not attended services provided at The Radix Pain & Rehabilitation Centre in the past 3 years.
